Summary:
Around 20% of people report surgery for osteoarthritis (e.g. knee replacement) does not improve their pain or ability to do daily activities. Learning mindfulness has been shown to help some people experience less pain and to be more active at 6 weeks and one year after surgery. But we do not know the best way to teach mindfulness in the NHS or if people will be willing to do it.
M-POWER tests the feasibility of delivering a large randomised controlled trial comparing three different ways to learn and practice mindfulness before joint surgery. 99 patients from four different hospitals will be recruited from surgical waitlists. They will be randomised to learn and practice mindfulness before surgery in the following ways:
1) 8-week mindfulness course and 10-minute daily mindfulness practice
2) mindfulness leaflet and 10-minute daily practice
3) mindfulness leaflet and 1-minute daily practice.
We will ask participants about pain, activity and mental health before and after learning mindfulness and three months after surgery. We will interview some participants to ask about being in the study. If we can deliver the mindfulness programmes, patients are willing to take part and say it is worthwhile, we will run a larger trial. A trial will tell us which mindfulness programme is the most helpful and cost-effective.
Sample size:
99 patients will be recruited from four NHS hospitals in the United Kingdom (Royal National Orthopaedic Hospital, Norfolk and Norwich Hospital, University Hospital Coventry and 糖心TV, James Paget University Hospital)
Primary outcome:
Feasibility outcomes such as recruitment, retention, adherence to the intervention and completion of outcome measures will be recorded to determine if progression to a larger trial is warranted.
Duration:
Recruitment begins in December 2026. Data collection will be completed in April 2028.
